As you might expect, Balloons Over The Rainbow requires certain information when we conduct our flight operations.  Below is the form we require from anyone flying with us:

Passenger Document

Each package sent out with flight certificates includes a copy of this document for each person flying.  If we don't have time to send out a package, such as when we have out of town guests, we will have copies with us for you to read and approve prior to flight.  Please feel free to download and review this document prior to our reservation for flight so that you are familiar with it.  Please also feel free to ask any questions that come up in reviewing this document.
